Home Support-Agency Per Diem
DSP’s Home Support-Agency Per Diem program provides comprehensive residential support to individuals residing in our agency operated homes. Our homes are staffed by caring and compassionate Direct Support Professionals who are always available (24/7) to respond immediately to the requests/needs for assistance from the individuals we serve.
Our residential program is highly individualized to match each individual’s goals and desired outcomes and assist them with acquiring, retaining or improving skills related to living in the community. DSP’s home settings are integrated in and support full access to the broader community, with services designed to ensure individuals receive opportunities to seek employment and work, control personal resources and engage in community life to the same degree as individuals without disabilities.
DSP staff work in collaboration with members of the individual’s planning team, including family members, other service providers, and members of the broader community, to facilitate opportunities and encourage development of a full life in the community, based upon what is important to and for the individual, as documented in their Person-Centered Service Plan (PCSP).
Individuals will receive all needed support from DSP staff to learn to use and apply personal initiative and maximize their ability to direct and exercise personal autonomy in the provision of services. Our residential programs ensure individuals have flexibility in setting their personal schedules and in making informed choices about their involvement with people, places and activities at their home and in the community.

Emergency Transitional Housing
DSP provides Emergency Transitional Housing (ETH) to any individual aged eighteen (18) and older who has been found eligible for adult Developmental Services through the Office of Aging and Disability Services (OADS). ETH is used in crisis situations when an individual’s needs cannot be met within their current living arrangements. This can include situations such as, but not limited to, when an individual is homeless, stuck in the emergency room, and/or a home provider cannot meet the individual’s current needs. Our ETH program provides a highly supportive and supervised residential setting where an individual can stabilize and return home or move to another location. DSP’s ETH staff ensure individuals receive all needed support in crisis planning and stabilization, training/assistance in daily living skills, medication administration, transportation and coordination with their Planning Team to ensure a successful transition to a long-term residential placement.
ETH can only be approved by the OADS Crisis Prevention and Intervention Services (CPIS) team. If you require ETH, please reach out to your Case Manager to submit an ETH request to OADS CPIS. If you require further assistance, please reach out the OADS CPIS team by dialing the Toll-Free Crisis Hotline.
